Buccal fat
Buccal fat is a deep pad of fat in each cheek, between the cheekbone and the jaw, beneath the chewing muscles. Forums treat removing it as a shortcut to hollow cheeks. Most cheek fullness in a photo is overall body fat and age, not this pad, so BecomeTen treats it as workable through body composition and never recommends the surgery.

What is buccal fat?
The buccal fat pad is a lump of fat deep in the cheek, between the cheekbone above and the mandible below, under the chewing muscles. Everyone has it, it is larger in infants, and it tends to thin with age, which is part of why older faces look hollower under the cheekbone. It is not the fat you can pinch on the surface of the cheek. That is subcutaneous fat, a different layer, and the one that rises and falls with body weight.
Why do forums obsess over buccal fat removal?
Because the surgery is easy to explain and the before-and-after photos are dramatic: an incision inside the mouth, the pad comes out, the cheek hollows. Forum threads treat it as the shortcut to a lean model's midface. What they leave out is that the model is lean everywhere, that the after-photo is lit from above to deepen the hollow, and that the pad keeps thinning with age, so a hollow that looks sharp at twenty-five can look gaunt at forty-five.
What a photo can and cannot show
A photo shows cheek fullness. It cannot say which layer it comes from: the deep pad, the surface fat that tracks body weight, water retention, or the light. Most of the time the answer is body fat and age. A 2009 paper on facial adiposity found raters read the fatness of a face from its shape and used it as a cue to health, and a 2010 study showed the cues are quantifiable: cheek width and lower-face roundness track measured body weight closely. Neither study needed to know anything about the buccal pad.
Is buccal fat workable or structural?
The pad itself is neither, in BecomeTen's terms: it is not bone, but nothing non-surgical removes it, and it is not rated on its own. What the report rates is cheek fullness and lower-face definition, and those are tagged workable, because the layer that changes them is body fat. A person who drops from a soft body composition to a lean one usually sees the cheek hollow appear without anyone touching the pad. What actually changes how buccal fat reads?
- Body fat. The largest lever by far, and the only one BecomeTen will put in a plan. Cheek fullness follows overall body composition, on the way down and on the way back up.
- Age. The pad thins over decades and the surface fat shifts downward. Fullness at twenty is not the fullness you will have at forty, with or without any intervention.
- Lighting. Light from above deepens the hollow under the cheekbone; flat light from the front fills it. Every surgical after-photo uses the first kind.
- Water, salt and expression. A salty meal or short sleep puffs the cheeks for a day, and a slight smile rounds them. Neither is your anatomy.

Is buccal fat the reason my cheeks look full?
Probably not the main one. Cheek fullness in a photo tracks overall body fat and age far more than the deep pad, and a photo cannot separate the layers anyway. If the face is soft everywhere, the pad is not the story. BecomeTen tags cheek fullness workable for that reason and points the plan at body composition.
Does buccal fat go away when you lose weight?
The deep pad shrinks a little with weight loss but mostly stays; the surface fat over it is what drops. In practice that is enough: the hollow under the cheekbone appears in most people as they get lean, without touching the pad. A lean person who still has full cheeks is the minority case, and the report says so.
Why do buccal fat removal results look so dramatic online?
Lighting, timing and selection. The after-photos are lit from above to deepen the hollow, taken once swelling has settled, and posted by people who were already lean. The same person photographed in flat light at a higher body fat would show far less. BecomeTen does not recommend the surgery at any tier.
